Disaster preparedness survey

To take the survey, click here. Please help us by promptly completing the survey. It should only take 5 to 10 minutes to complete.

What’s in it for you? Besides helping us tailor our training and CE opportunities, everyone who completes the survey will be entered into a drawing for a 50% discount on one of WSVMA’s one-day CE seminars.

WSVMA and the Washington State Animal Health Foundation were awarded a grant from the American Veterinary Medical Foundation for initial disaster and emergency preparedness planning. Your responses to the survey will help determine current resources within our veterinary community, what resources need to be developed and any training that should be developed.

The survey will be sent out in a SurveyMonkey format.  If your e-mail is not listed in WSVMA’s database or you are unable to access the survey, you can request that a copy be mailed or faxed to you for completion.  To receive a copy of the survey, contact Dr. Kathleen Connell, WSVMA Disaster Grant Program Director, (360) 753-7080 or drkathleenconnell@msn.com, or the WSVMA office, toll free (800) 399-7862 or info@wsvma.org.

Information provided through the disaster and emergency preparedness survey is for internal WSVMA use only. Results will be discussed at membership meetings of the 14 local VMAs and will be available on WSVMA’s website and in Sparks and Insight.

Thank you in advance for your help in completing this important survey. Your responses are important.
